Policy Requirements
- Must be aged between 18-64
- Been working for a long period of time and work a certain amount of hours a week
- Be employed in the UK.

This means there is no need to fill out any questionnaires relating to your medical history, and your doctor will not be requested to write a report on any medical treatment you may have received.
